Specimen kits are packaged to your specifications and can be easily ordered by one of the following options:
1. When submitting a specimen to our lab, fill out the bottom portion of the test requisition that contains the order section.
2. Call us toll free at 800-527-6446 ext. 198. Available 24 hours a day.
3. Visit our web site and submit the online Order Supplies form.
Specimen Transport
Specimens are shipped to our laboratory via Federal Express
for speedy delivery and tracking capability. A courier
service is utilized for local accounts.
